Woodlands Place Rehabilitation Suites is focused on rehabilitation opportunities. We offer a wide range of rehabilitation services that are tailored to each resident's needs. The Rehabilitation Department offers services from certified and licensed professionals in physical therapy, occupational therapy and speech pathology. These therapists create individualized therapy plans to promote wellness and recovery. The department's goal is to facilitate independence for a successful return to the resident's prior level of functioning.

Woodlands Place Rehabilitation Suites has the latest therapy equipment, and offers extensive therapies to meet virtually any rehabilitation need.

We offer services required to help those in either short-term or long-term complex rehabilitation. Our qualified, licensed therapists provide comprehensive rehabilitative treatment in three primary disciplines which include physical, occupational, and speech therapy.

Woodlands Place Rehabilitation Suites is devoted to discovering each individual's full functional capabilities as well as aiding and achieving each resident's highest practical level of independence.

Individualized therapy and treatment is provided by skilled clinicians in our spacious and fully equipped therapy gym. Our facility is designed with the very latest therapy assessment, monitoring, and treatment modalities. Our rehabilitation strategy is tailored to the individual. We do this by simulating an environment that resembles home as much as possible in our ADL kitchen, bathroom, laundry areas, transition suite, and mobility garden.

Our dedicated Short-Term Rehabilitation Program at Woodlands Place Rehabilitation Suites consists of an interdisciplinary team that evaluates each resident's unique situation and works to create a cohesive and individualized plan for success. During treatments, our therapists will work to improve independence, strength, and confidence so our patients can transition to the least restrictive environment upon discharge with the goal of returning to their prior living environment.

At Woodlands Place Rehabilitation Suites, 24-hour skilled nursing care is provided by a team of nurses and nursing assistants. Our nursing staff includes Registered Nurses, Licensed Practical Nurses, and Certified Nursing Assistants. Our wound care nurses and restorative aides can help residents meet their highest level of functioning. Our case management team shares the same passion to help residents return to their community and/or achieve their highest level of functioning.

Our Long-Term Care Program was designed to assist those who may benefit from 24-hour nursing, meal assistance, medication management, as well as other activities required for daily living. We strive to maintain a very comfortable living environment for our residents with an emphasis on health and wellness.

The resident's interdisciplinary team supports and designs care plans that seek to heal our resident's wounds under the direction and guidance of an independent physician. Our licensed wound care nurses work seven days a week for residents requiring treatment for complex wounds, including but not limited to post-operative wound sites, diabetic, and decubitus wounds.

Common Diagnoses

We are pleased to offer a Transition Room where residents can recover their skills in cooking, bathing, and movement abilities in a home-like setting. This prepares our residents in short-term care to transition home.

Our private Transition Rooms are fully equipped with a refrigerator, microwave, dining area, shower, and bathroom to help our residents regain confidence in an independent setting. This also allows staff of Woodlands Place Rehabilitation Suites to assess the resident's ability to care for himself or herself. Our team of nurses and therapists work hard to identify and assess any areas of concern in a home-like environment prior to discharge. Living in the transition room prior to returning home can help increase confidence to promote a safe return home.

Woodlands Place Rehabilitation Suites' Dining menu follows a liberalized philosophy supported by the American Dietetic Association and the American Diabetes Association. Our Culinary Services Director will meet with you to discuss any specific food restrictions and/or personal preferences.

We provide breakfast, lunch, dinner, and snacks to all of our residents. We encourage residents to dine in the beautiful and spacious dining areas to encourage socialization. However, if residents prefer to eat in the comfort of their room, they are more than welcome to do so.

We pride ourselves in our ability to provide a person-centered approach to our residents' dining experience. Residents are served restaurant style and their individual selection is provided by our staff at meal time. All meals are prepared fresh daily and specifically created to suit all residents' individual needs, preferences, or modified texture diets.

Activities help bring members together socially and can improve their quality of life. Woodlands Place Rehabilitation Suites is very proud of the various recreational activities it offers for its residents. Each resident is individually assessed to provide a program of activities that addresses his or her physical, spiritual, emotional, recreational and social needs. Our certified Activity Director and volunteers encourage many levels of independence.

Our activity programs often include field trips into the community. Trips can include places such as restaurants, movies, malls, and community events. The activities department can also help provide personal shopping assistance and the facility provides transportation to and from the various destinations.

Religious and spiritual services are also available, and our activities staff will assist if a specific religious or spiritual need is requested.
